Your brain actually cannot run only on ketones. Even in the deepest ketosis, you can only run on about 70% ketones. You still need glucose, which means your body still has to make glucose by tearing down amino acids and spiking cortisol and adrenaline. You cannot function without glucose. So either you eat it or you make it through catabolic processes that involve stripping muscle and bone tissue of amino acids.
